Monday-Thursday: (9:00am – 5:00pm)
Friday: (9:00am – 12:00pm)

Insurance

We participate and bill most major insurance plans. Please notify our office of your insurance at the time of your appointment and we will verify participation in your plan.

Insurance Billing Policy: Please bring your current insurance card to your appointment. We will gladly file all insurance claims for you. Insurance co-pays are expected to be made at the time of your appointment.

Payment Options

For your convenience, we accept cash, personal or bank checks, debit and credit cards. For any procedure that is not billable to insurance we will provide you with a detailed fee estimate at the time of your appointment. Need help with payments? Apply for Care Credit Today!

Cancellation Policy

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.

Affiliated Hospitals

Dr. Birndorf maintains privileges at both United Hospital campuses, St. Catherine’s and Kenosha. He also has privileges at Aurora Medical Center in Kenosha, the Aurora Surgical Center in Racine and Wheaton Franciscan Hospital in Racine.

Emergency Instructions

Dr. Birndorf is available at all times for his established patients. To contact him after hours all that is necessary is to call his office number.
